Right to Access

You have the right to inspect and obtain a copy of your health information maintained by Contour Med Spa. To request access, submit a written request to our Privacy Officer at hello@contourmedspa.com.

You have the right to request that we amend your health information if you believe it is inaccurate or incomplete. Submit amendment requests in writing to our Privacy Officer.

Right to an Accounting of Disclosures
